Our commitment to your privacy
Long Point Counseling, LLC and Jeff Marcino, Psy.D, LPC are committed to protecting the privacy of your health information (protected health information, or PHI). This notice describes our practices regarding your information and your rights.
How we may use and disclose your information
- Treatment. We use your information to provide and coordinate your care.
- Payment. As a private-pay practice, we use limited information to bill you and process payment. We do not file insurance claims unless you request documentation (a superbill) to submit yourself.
- Health care operations. We may use information for quality, training, scheduling, and practice administration.
Uses and disclosures that require your written authorization
Most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of information require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted in reliance on it.
Uses and disclosures permitted without your authorization
The law permits or requires certain disclosures without your authorization, including: when required by law; to prevent a serious and imminent threat to health or safety; for the reporting of abuse, neglect, or domestic violence as required by law; for health oversight, judicial, or law enforcement purposes as permitted by law; and to coroners, medical examiners, or for workers’ compensation as required.
Your rights regarding your information
- The right to request access to and a copy of your records.
- The right to request an amendment of your information.
- The right to an accounting of certain disclosures.
- The right to request restrictions on certain uses and disclosures.
- The right to request confidential communications by alternative means or at an alternative location.
- The right to a paper copy of this notice.
Our duties
We are required by law to maintain the privacy of your information, to provide this notice of our duties and privacy practices, and to follow the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
